About The Medical Records

All medical records available to you are reproductions of those provided to me upon my request for certified copies.

I have made some changes which I believe you will accept as non-essential.

For example, I have tried to remove JA's medical record number from the hospital chart. I think that you will agree that this is a non-substantive alteration.

The medical records can be broadly classified into two groups: annotated and un-annotated.

I recommend that you look first at the un-annotated version of each record from the hospitalization. I suggest this to prevent any bias that you might think I possess.

Then look at the annotated versions. In them, I comment on certain points, but by no means all of them. I also try to explain some of the medical terms which might be unfamiliar to you.

Remember, this is a novel. Only the un-annotated medical records and legal filings can be considered as accurate representations of what is claimed to be true.

Some of these files are large. A fast internet connection helps.

It is likely that you will have to "play" with the settings on your PDF reader to see the complete pages. In general, you will have to decrease the zoom (e.g., going from 100% zoom to 33% zoom). This is more likely to be true for the annotated records than it is for the un-annotated records.

Here is the link to the un-annotated hospital record (the full record is about 61 Mb and contains 110 pages).

Here is the link to the annotated Discharge Summary (the annotated summary is about 9 Mb. It comprises 4 pages of medical records and annotations). This the work of Dr. Layfer.

Here is the link to the annotated Patient Progress Form (the annotated progress form is about 560 Kb. It comprises 1 page of medical records and annotations). This displays the combined efforts of the nurses and Dr. Ibrahim.

Here is the link to the medical records maintained by Dr. Leyenson. These are linked separately since they comprise both office records and records from the hospitalization. A comparison of the two might be enlightening. (the annotated office record is about 4.3 Mb and comprises 13 pages of medical records and annotations and the annotated hospital record is about 1 Mb and comprises three pages of medical records and annotations)

Here is the link to the annotated Surgical Consult Form. This was written by Dr. Velasco and Dr. Rastatter. As you may recall, Dr. Velasco was the Chief of General Surgery. You may find the quality of his work interesting. (the annotated surgical consult form is about 876 Kb. It comprises 3 pages of medical records and annotations).
